The high quality nursing care at Oaknoll is based on cutting edge principles and utilizes the latest trends in the field. We have a great team of certified staff at all levels of care to make sure that our residents are taken care of 24-hours a day seven days a week.
Meet our Director of Health Services
The Oaknoll nursing care team is led by Kim Bergen-Jackson. Kim is responsible for all nursing and assisted living services. Through her pursuit of a Ph.D, Kim has developed professional ties to the University of Iowa, and she is able to use these relationships and her education to implement new ideas and train her staff on the latest innovations in the field of gerontology. She ensures evidence based practices and a standard of care to Oaknoll that is measurable and supportable. She also oversees admissions to Oaknoll, outreach programs and research. Kim has a wonderful support team at all levels of care, which includes:
Assistant Director of Nursing, who manages day-to-day nursing care services and plays a vital role in assuring the delivery of quality care and resident-focused assistance in the Health Center.
Assisted Living Nurse Coordinator, who is responsible for overseeing the Assisted Living staff and all Assisted Living services. She also oversees individualized service plans of each resident which includes goals and approaches intended to keep the residents as independent as possible while providing superior nursing care.
Care Plan/MDS(Minimum Data Set) Coordinator, who is responsible for coordinating and completing care plans with all of the Health Center residents and documenting the goals and nursing plans decided on during quarterly care plan meetings attended by the resident, their family, and the interdisciplinary team. From the time a person is admitted to the Health Center our staff is getting to know the resident and their needs. From our assessments we develop an individualized care plan.
Other important members of the care plan team include the Care Plan Coordinator, a Registered Dietician, the Recreational Therapist, and the Social Workers. During the conference the care plan will be reviewed and updated. The care plan consists of goals and approaches that are intended to keep the resident as independent as possible while providing superior nursing care. Residents are encouraged to attend the care plan conference. Residents and their families can request a care plan meeting at any time.
Nurses/Certified Nursing Assistants
The Health Center is staffed with nurses 24 hours a day. The nurses are responsible for coordinating all nursing activities in the Health Center during all shifts. The nurses work together with the Certified Nursing Assistants (CNA’s) to provide for the physical, emotional, and overall well-being of the residents. The nurses administer medications according to each individual residents doctor’s orders. Medications are ordered from a contracted pharmacy.
Assisted Living Staff
Assisted Living is staffed 24 hours a day with a team of Nurses, Certified Medication Aides, Assisted Living Techs and Certified Nursing Assistants. Nurses, Certified Medication Aides and Assisted Living Techs administer medications, treatments and assisted with personal cares according to physician’s orders and an individualized service plan. Certified Nursing Assistants assist with personal needs and cares to provide for the physical, emotional and overall well-being of the residents. There is at least one nurse, Certified Medication Aide or Assisted Living Tech to manage medication and treatment needs at all times of the day and night.
There are a number of people available to meet specific needs of Health Center residents. These consultants include: speech therapist, physical therapist, occupational therapist, a certified dietician, dental services and podiatry.
